Health information exchange is very important to the populations that South Country Health Alliance serves in rural Minnesota. In these rural communities there are limited health care resources for our members, and because of this members travel far distances to seek the care they need. Members with multiple comorbidities seek care from several providers and multiple care centers. Not only primary specialty and acute care, but from the entire continuum of care. Health information exchange helps us bridge these gaps in care and makes it easier for our members to get the care they need. With health information exchange we’re able to improve population health through better care coordination and create better overall health outcomes and experiences for our members.
